For manual applications, copies of the FDWs passport and boarding pass are required for verification.
Please note that applications for the waiver of levy must be within a year after the end of the month in which the levy was paid. For example, to claim for a levy refund for November 2006 (paid in December 2006), employers must apply before end of December 2007.
